Privacy and HIPAA Compliance

The privacy of your personal and health information are very important to us at the Tobacco QuitLine. We use your personal information to assist you in quitting smoking and to improve our program. We will not share your information with anyone or any entity not connected to our smoking cessation program, except as described in the Notice of Privacy Practices, below.

Notice of Privacy Practices for Tobacco QuitLine Vendor

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please READ it carefully.

The Medica Tobacco Cessation QuitLogix Program is operated by National Jewish Medical and Research Center ("National Jewish"), a health care provider that has developed a program to assist individuals to reduce and discontinue the use of tobacco products, to provide individuals with assistance and support to quit using tobacco products.

Federal privacy regulations require National Jewish to abide by the terms of this Notice of Privacy Practices. This Notice of Privacy Practices describes how the staff of QuitLogix may use and disclose callers’ protected health information. Protected health information (PHI) is information about Tobacco QuitLine participants’ health and the healthcare services they receive. We are required by law to keep this information private. We are also required to provide callers with this notice of our privacy practices

The staff of Tobacco Quitline will use PHI to advise callers on how to reduce their use of tobacco products. National Jewish, and the staff will not use PHI to communicate with other healthcare providers, such as the callers’ referring physicians unless we have the caller's authorization. We will not use PHI for any other purpose than advising participants on the way to quit tobacco products, quality review, and staff training. We will use non-identifying information for data collection, analysis and reporting to the Medica Tobacco Cessation QuitLogix Program.

Sometimes, applicable law requires us to disclose participants’ PHI. We notify participants of these disclosures when the law requires us to do so. We may disclose participants’ PHI to: public health agencies responsible for prevention or controlling disease or injury, National Security and Intelligence agencies; health oversight agencies; Food and Drug Administration (FDA) and law enforcement agencies.

We will not disclose a participant's PHI to members of their family or to any other person unless the participant asks us to do so.
